What immunizations students needChildren entering pre-kindergarten
- Three doses of Hepatitis B vaccine;
- Four doses of DTaP (diphtheria, tetanus, pertussis) vaccine;
- Three doses of polio vaccine;
- One dose of MMR (measles, mumps, rubella) vaccine;
- Four doses of Pneumococcal Conjugate (children 15 months through 5 years of age who have received 4 doses of Prevnar should receive one dose of Prevnar 13 to elicit immune response to the six additional strains.);
- Four doses of Hib or completed series;
- One dose of varicella (chickenpox) vaccine or a statement signed by a doctor stating that the child has a history of chickenpox.
- 5 doses of DTaP (diphtheria, tetanus, pertussis) vaccine;
- 3 doses of Hepatitis B vaccine;
- 2 doses of MMR (measles, mumps, rubella) vaccine;
- 4 doses of polio vaccine;
- 2 doses of varicella (chickenpox) vaccine or a statement signed by your child’s doctor stating that your child has a history of chickenpox disease.
- 1 dose of Tdap (tetanus, diphtheria, pertussis) vaccine diphtheria-tetanus containing vaccine.;
- 1 dose of meningococcal conjugate (meningitis) vaccine.
- 1 dose of HPV (human papillomavirus) vaccine.
>2 doses of HPV (human papillomavirus) vaccine.
Children entering 12th grade
- 1 dose of meningococcal conjugate (meningitis) vaccine - booster dose.
